NOTE: The USA Roller Hockey general liability policy covers only those sanctioned roller hockey events of USA Roller Hockey and its sanctioned organizations. Only activities that take place during games and practices explicitly approved by a sanctioned organization are covered under this policy.

Participant Accident Policy

  • Carrier – Berkley Life & Health Insurance Company
  • $25,000 Accident Medical Expense Benefit
  • Deductible - $500
  • $1,000 Durable Medical Equipment Benefit
  • $1,000 Outpatient Prescription Benefit
  • $1,000 Physiotherapy Benefit ($50/per visit)
  • $1,000 Chiropractic Benefit ($50/per visit)
  • $5,000 Accident Death & Dismemberment ($10,000 Aggregate)

Commercial General Liability Policy

  • Carrier - Philadelphia Indemnity Insurance Co.
  • $3,000,000 General Aggregate Limit
  • $1,000,000 Personal and Advertising Injury Limit
  • $1,000,000 Each Occurrence Limit
  • $300,000 Damage to Premises
  • $25,000 Abuse and Molestation – Each Occurrence ($50,000 aggregate)
  • Included Participant Legal Liability
